What We Treat

ACL tear treatment in Washington.

The pop you heard, the swelling that came in within the hour, and the long list of things people have told you since. Whether surgery is ahead of you or behind you, it starts the same way: an honest measure of what that knee can and cannot do right now.

Sound Familiar?

Does this sound like you?

  • The pop you heard, and a knee that filled up within the hour
  • The knee giving way when you pivot, months after everyone said it was healing
  • Going down stairs sideways, because straight ahead does not feel safe
  • Standing with all your weight on the other leg without noticing you do it
  • One thigh that has visibly gone smaller than the other
  • Cleared by everybody, and still not willing to cut at full speed on it

Why It Happens

The ligament tore. The leg has to be rebuilt around it.

Your ACL is a short, strong band inside the knee. Its job is to stop your shin sliding forward and to stop the knee twisting apart. Most ACLs go without any contact at all: you plant, you decelerate, the knee rolls inward, and the rest of you keeps turning. What hands the ligament that much load is rarely just bad luck. A hip that does not control rotation, a quad that switches off, an ankle that cannot bend far enough, all of it pushes the job onto the ACL.

Which is why an ACL is never only a knee problem. Surgery can replace the band. Nothing about the operation retrains the hip, wakes the quad back up, or changes how you land. That part is the actual work, and it is the difference between a knee that holds and a knee you never quite trust again. So we measure where your leg is now, both sides compared, and build from there.

The Honest Truth

A quiet knee is not a ready knee.

If you keep waiting

  • The quad keeps shrinking, and quad strength is the thing the knee leans on most
  • You teach yourself a limp your body keeps long after the pain is gone
  • You go back to sport on a knee that passed nobody’s test, which is how second injuries happen

If you start this week

  • You get an honest measure of the knee, both legs compared, at visit one
  • If surgery is coming, you go in stronger, and stronger going in is easier coming out
  • You get a plan with an endpoint, and a clearance you actually earn

Nobody regrets starting too early. Plenty of people here say some version of I should have come in sooner. Almost nobody says the opposite.

How We Fix It

The same four phases. Built for a knee you can trust.

You are the one who shows up for the team, the office and the family, which is exactly why the knee got put off. Every plan here has an endpoint, and for a knee the endpoint is a test, not a date. Timelines follow your surgeon’s protocol.

Weeks 0–2

Pain

Get the swelling and the fear down.

Hands-on work and carefully chosen early motion settle the joint and wake the quad back up, whether you are heading for surgery or fresh out of it.

Weeks 2–12

Prime

Straighten it, bend it, own it.

Full extension comes first, then flexion and a normal walking pattern, because every later stage is built on the range you win back here.

Months 3–6

Perform

Rebuild the leg, not just the knee.

Quad, hamstring, glute and calf get rebuilt side against side, then we add hopping, landing and change of direction under control.

Months 6+

Prevent

Pass the test, then go.

Strength and hop testing compares your two legs under real load. You are cleared when the knee earns it, and you leave with what keeps it that way.

My knee and associated muscular pain bothered me for over five years, resulting in a reduction in my ability to continue my exercise regime of cycling and having to stop running. My physiotherapist developed a tailored treatment plan including dry kneedling, manual therapy and at home exercises to help me resume a lifestyle that I enjoy.

Questions, Answered

What people ask before booking for an ACL tear.

Not always, and that call belongs to you and your surgeon, not to us. Some people, especially those who do not need to cut and pivot, do well with rehab alone. Others need the reconstruction to get back to their sport safely. What we can do is measure the knee honestly, tell you what it is doing without a graft, and work alongside your surgeon either way. Patients here have been sent straight to an orthopedist because that was the right call, and we will tell you plainly if that is you.

You can usually start before the operation, and that is the most underrated part of ACL recovery. Going in with a straighter, stronger knee makes the months afterwards noticeably easier. After surgery, everything follows your surgeon’s protocol. The reason to work one-on-one with a licensed physical therapist, every visit, is that somebody is watching the load instead of handing you a sheet. You should feel the leg working hard. You should not feel the graft.

Ask us to check, and we will look before you owe anything. Request a free benefits check and we will go through what your plan actually covers for physical therapy, deductible included, so nothing lands as a surprise. You can also start with a free 30-minute Discovery Session or a free 15-minute phone call, both before any commitment. Plenty of people decide it is worth it once they see a plan with an endpoint instead of an open tab of visits.
